Medicare Part B, most Medicaid programs and most private insurance plans use HCPCS (Healthcare Common Procedure Coding System) for enteral feeding devices, grouping products into categories. These codes, categories and the Medicare Part B allowable rates are listed below.
Enteral Pumps/Devices HCPCS Codes and Rates
Enteral Feeding Supply Kits (Pump, Gravity or Syringe/Bolus)
The HCPCS code for pump supply kits is B4035 and may be reimbursed by Medicare at $11.30 per day.
The HCPCS code for gravity supply kits is B4036 and may be reimbursed by Medicare at $7.76 per day.
The HCPCS code for syringe/bolus supply kits is B4034 and may be reimbursed by Medicare at $5.93 per day.
Nasogastric Tubes
The HCPCS code for Nasogastric tubes with stylets is B4081 and may be reimbursed by Medicare at $20.96 per tube. Tubes are expected to last 30 days.
The HCPCS code for Nasogastric tubes without stylets is B4082 and may be reimbursed by Medicare at $15.59 per tube. Tubes are expected to last 30 days.
G or J Tubes
The HCPCS code for standard Gastrostomy or Jejunostomy tubes is B4087 and may be reimbursed by Medicare at $34.59 per tube. These tubes are expected to last 90 days. This code changed from B4086 to B4087, effective January 1, 2008.
Low Profile Gastrostomy and Jejunostomy tubes are assigned HCPCS B4088 and may be reimbursed by Medicare at $34.59 per tube. These tubes are expected to last 90 days. This code changed from being included in the standard G tube category of B4086 to being separated with it's own code effective January 1, 2008.
Enteral Feeding Pumps with Alarms (all Abbott Nutrition Pumps have alarms)
The HCPCS code for all enteral feeding pumps with alarms is B9002. Medicare uses modifiers to determine how the pump is supplied.
B9002 NU is used to indicate the purchase of a new enteral pump with alarm and may be reimbursed at $1,188.74.
B9002 RR is used to indicate the rental of an enteral pump with alarm and may be reimbursed at $115.13 per month. There are additional HCPCS modifers to indicate that an enteral feeding pump is being rented and the specific month of rental as well as modifiers that indicate the decision of the patient regarding the purchase option. These modifiers can be found in a Medicare Supplier Manual or by calling the Abbott Nutrition Reimbursement HelpLine at 1-800-558-7677.
B9002 UE is used to indicate the purchase of a used enteral pump with alarm and may be reimbursed at $891.55.
